Why does this trigger cause the sound file to double play back?

Event:

When Death Phantom Notices a target in range.

Condition:

None

Action:

Set up all volume channels for speech
Cinematic Transition fron Unit to Player Group 2(Blue), from Death Phantom, named Death Phantom, Play Shadepissed2(gen)
Reset all volume channels to 100%
Turn this trigger off

Whenever I go get close to him it plays the sound not once but twice and happens in numerous places in my campaign, but for some reason only on patciular people... This event it ALWAYS happens...
Yet the level prior to this I have the EXACT same trigger set up for something else and it works perfectly.
