Item Cooldown Cannot Seen

First.My english is terrible

Ehm..

I have created item from crystall ball and i changed item icon i can cast spell but item cooldown and item mana cannot seen how can i fixed it?

You gave your item an active ability that has a mana cost and a cooldown, and some passive abilities, right? In that case, make sure the active ability is the first one on the item's abilities list, if there's any passive abilities on the list before it, the item won't show cooldown.
