What did you make this in? TVertices should be between 0 and 1, and some of yours are more than 1. If this was exported from 3ds Max, I'd try collapsing the stack on your mesh and exporting again. |

Are you using dex or Blizzard's one? On the modify window, there's that stack of stuff, one of which is probably unwrap. Right click on the list and go to "Collapse all" or something. |

Could always put WrapWidth and WrapHeight on the textures and see what comes out? |
