Problem with my quest system

After player types Accept if can't get past creating the quest item in the quest menu.
I've attached my map.

If someone could please help me out.
Its suppose to show a seperate !, ?, or (yellow) ? for each player.
Quests are basicly seperate for each player.
Even quest log is suppose to be seperate.
Please help me get this system working right.

Any help is welcomed
