Hero dies when picking up an item (sometimes)

I'm not sure if this has been addressed, but sometimes when picking up items, they die, especially if they stand right on top of it. This only seems to happen with some items, and only sometimes. I've had one player pick up an item and die while another picks it up fine. Has anyone encountered this before?

Never ever seen it before, and i dont think its possible, maybe you have or they had a trigger that does that?

This can happen if the item effects the health of the unit, i.e negative hp gain/regeneration.
