Changing Items with Triggers?

I tried searching the forums for information on this, but I only found recipe related things. What I'm asking about is this: is it possible to change the Ablilities and Tooltips of an item by using triggers. To make myself clear, here's an example. Let's say a hero has a "Belt of Giant Strength +6". He then picks up a custom item "Belt Modifier +3" which adds a "+3 Strength" ability to the "Belt of Giant Strength +6" and changes its name and tooltip accordingly. Now, most people will say, "Why not just make a new item and detect when the hero has both, and replace, easy." But what if you had multiple modifiers for a single item? The possiblities add up fast, and I would end up making hundreds of items. Any help would be appreciated, and if this is confusing, I can elaborate more.

No, it would be wonderful if it was possible, in fact I believe it's one thing that is most desired, but that's beyond the current object-oriented capabilities of Warcraft 3. Look at the Socketing feature of Vexorian's InvX, that's the closest you'll probably find to what you're looking for.

My map has a random item bonus generation system. The item description is shown via multiboard, its probably the best method to do it, since item tooltips cant be modified with triggers.
